Protecting your privacy is very important to Healthcare Underwriters Group of Florida (HUFL) and Global Insurance Management Company, LLC (GIMC). We want you to feel secure when providing information to us online. To let you know how we use information collected through HUgroupsFL.com (the “HUFL website”), we have established the following Privacy Policy (“Privacy Policy”). Since this Privacy Policy may change from time to time, please refer to it regularly.


1. INFORMATION WE COLLECT AND HOW WE USE IT
 a. Information Collected Automatically. You can visit many areas of the HUFL website anonymously without the need to become a registered user. In such case, our web servers may still automatically identify and collect information such as the IP address or name of the domain you used to access the HUFL website, the referring URL, and the type of browser and platform used to access the HUFL website. We typically use this data to monitor our system performance and/or to make it easier and more convenient for you to navigate and use the HUFL website.
  
b. Cookies. The HUFL website does not employ the use of cookies.
  
c. IP Addresses. We may collect IP addresses for the purposes of system administration and to audit the use of our website. Each time you log onto our website and each time you request one of our pages, our server logs your IP address.
  
d. Information You Submit. We may collect personally identifiable information when you communicate with us by e-mail, by phone, in writing, in person, or when you submit an application for coverage. We use the information we collect to respond to your inquiries, underwrite your policies, ensure proper billing, service your accounts, and offer you other products and services that we believe may suit your needs.
  
e. Registration. Some of the areas on the HUFL website require you to register with HUFL before you are granted access to them. These areas represent secure areas of our website that we use to provide or receive personally identifiable or other sensitive information. The amount of personally identifiable information that we ask you to provide depends on the product or service that you have requested or are participating in. Typical registration may include, among other things, your name, practice type, street address, ZIP code, telephone number, and e-mail address. Should you decide to apply for an insurance product through our Online Application, you may be required to provide additional personally identifiable information during the underwriting process.

  
2. SHARING OF INFORMATION
 a. Non-Personally Identifiable Information. We may provide certain aggregate information we collect from you and our other customers or visitors (e.g., statistics about our customers, sales, traffic patterns and services) to third parties who can assist us in analyzing and understanding the information and help us to better provide online products and services. Should we share statistical information with third parties, it will not include any personally identifiable information.
  
b. Personally Identifiable Information. We do not sell your personally identifiable information to third parties. When you provide personally identifiable information to us, we may share it with our affiliated companies and third-party contractors who are subject to confidentiality agreements solely for the purpose of fulfilling your requests, carrying out our obligations as your insurer or offering our products and services.
Please bear in mind that anyone you provide with your log-in information will also have access to your account and personally identifiable information.
We may disclose your personally identifiable information to others if, required to do so by law or legal process, to respond to insurance claims or to protect the rights, property or safety of HUFL, GIMC or others. We also reserve the right to provide your registration, account and other personally and non-personally identifiable information to an acquirer of all or substantially all the assets of HUFL and GIMC so long as such acquirer has agreed to maintain your personally identifiable information in confidence and use and disclose it solely in accordance with this Privacy Policy.
   
 
4. SECURITY
 a. On-Site Security. HUFL maintains a number of safety practices to help ensure the continuing security and confidentiality of your personally identifiable information. We have physical security at the site of our servers, passwords to protect our databases, and virus/intrusion detection software. All HUFL and GIMC employees, who have access to, and are associated with the processing of personally identifiable information, are obliged to respect the confidentiality of your information. Employees who misuse customer or visitor information are subject to disciplinary action.
  
b. Online Security. Certain secure areas of the HUFL website use both Secure Socket Layer (SSL) encryption and authentication tools (User ID and Password) to protect the transmission of your personally identifiable and other sensitive information.
Please keep in mind that any e-mails you send to us and that we send to you are not necessarily secure during transmission. If your communication is very sensitive or includes highly confidential information such as premium or loss information, you may want to send it by postal mail instead.

 HUFL and GIMC reserve the right to modify or supplement this Privacy Policy at anytime. If we make any changes, we will update the HUFL website and Privacy Policy to include such change. Please review this Privacy Policy often to review any changes that may affect your use of the website. Your continued use of the HUFL website, once the revised Privacy Policy has been posted, affirms your agreement to such changes.
